MORE ABOUT REQUIRED SIGNATURES and SUBMITTING THE PROPOSAL ON-TIME

Your Internship Proposal form (the signed hard copy) establishes a particular work experience as the internship requirement of the HumBio major – thus the importance of the proposal being submitted to the Student Services office on-time.

Three signatures are required on the proposal: “Student Advisor” (SA or Student Services Coordinator), “Faculty Advisor” (FA), and “Internship Supervisor”. PLEASE DO NOT ASK YOUR FACULTY ADVISOR OR INTERNSHIP SUPERVISOR TO SEND AN EMAIL TO STUDENT SERVICES IN LIEU OF SIGNATURES ON THE INTERNSHIP PROPOSAL FORM. Obtaining the required signatures on the Internship Proposal form and submitting it on-time to Student Services, is the responsibility of the Human Biology student and a requirement of the internship, so being pro-active early is key and will save you time in the long run. Internship Proposals that are turned in later than the quarter following the completion of the internship will need a petition and an additional six page report on the internship experience.

Students undertaking non-remote internships are expected to complete the proposal and submit it to Student Services on time/before the internship begins with all three required signatures (using fax or scan as needed). Students can use the HumBio fax (650 725-5451) for internships within the United States.

About obtaining the Faculty Advisor signature if the FA is out-of-area: Students can use the HumBio fax (650 725-5451) to forward the proposal for signature to their FA who is out of the area. If the FA is at a remote location without a fax, the student can obtain the FA signature and turn in their completed proposal during the quarter following the internship provided the student received email confirmation from the FA that s/he will sign upon return (to campus) the quarter following the internship. (If the student is presenting his/her poster during that quarter, the proposal must be turned in by the day of the poster presentation.) The student should attach a copy of this email to the internship proposal.

About obtaining the Internship Supervisor’s signature: In order to obtain the Internship Supervisor's signature, a student may find it necessary to turn in the completed/signed proposal while the internship is underway (via fax, scanned email, or US mail). It is recommended that the student makes a photocopy of the proposal signed by the FA and an SA for their HumBio folder, before leaving for an internship out of the area.

If a student undertaking an internship at a remote location does not have access to a fax or scanner, s/he may turn in the completed/signed proposal upon return to campus the following quarter (if the student is presenting his/her poster during that quarter, the proposal must be turned in by the day of the poster presentation). It is recommended that the student makes a photocopy of the proposal signed by the FA and an SA for their HumBio folder before leaving for an internship out of the area.

JUNIOR and SENIOR Proposal Submission Deadline: A student undertaking the internship in JUNIOR year must submit the completed, signed internship proposal form to Student Services (Lia Cacciari) NO LATER than the quarter following the internship (by the day of the poster presentation).

A student undertaking the internship in SENIOR year must propose the internship and submit the completed, signed internship proposal form to Student Services (Lia Cacciari) NO LATER than two quarter's before the graduation quarter (i.e., Autumn quarter if intended graduation quarter is Spring).
